Start with the evidence
Open the attachment from the record being reviewed, not from a separate recent-files list. Compare its merchant, reference, currency and total with the surrounding record.
Work through the example
A USD32 record links to a USD23 receipt from the same shop. Similar filenames concealed a transposition. The mismatch should be resolved before the item is described as supported.
Make the next step clear
Correct the link through the established process and note the reason where appropriate. Check whether the displaced receipt belongs to another record so the fix does not leave a new unexplained gap.
Keep the source context
A readable source and a clear label serve different purposes. Keep the original document intact while adding searchable context around it. Avoid treating filenames, OCR output or a cropped image as more authoritative than the actual source.
